Nested emphasis now renders in markdownfix

Bold with italics inside and bold-italic now render correctly everywhere markdown is shown — posts, previews, and imports.
Found while previewing a Headway-hosted changelog on /try: their HTML converts to markdown with nested emphasis, and our renderer left literal asterisks behind. Fixed for all imports and hand-written posts alike.
